Description of Responsibilities
The Specialty Care Coordinator is responsible for managing and coordinating the care of patients receiving specialty and specialty lite medications. This role involves working closely with patients, payors, providers and hubs to ensure seamless access to medications, adherence to treatment plans, and optimal health outcomes.
Reporting Relationship: Specialty Pharmacist or Pharmacist-in-Charge (PIC)

Responsibilities of the Specialty Care Coordinator include the following:
Serve as the primary point of contact for patients, demonstrating a detailed knowledge of the patient care process. Participation in the daily patient care operations is expected including a basic understanding of specialty medications, routine prescriptions processess, communication with providers and communication with payors. Standards shall be adaptable to the daily ebb and flow of a specialty pharmacy, but shall reflect excellence throughout the process.
Enforcement of the highest communications standards is required. These standards shall include, but are not limited, to applicable verbal, written, and electronic communication and documentation. Each coordinator will display a positive attitude and build positive connections with patients and other key stakeholders.
Utilize electronic and/or digital technology in the provision of patient centered care. Maintain digital tools and platforms to enhance patient communication. Collect and analyze data to identify trends, improve patient care and support quality improvement initiatives. Ensure compliance with data privacy and security regulations in all IT and communication activities.
Establish and maintain strong relationships with providers, receiving and verifying comprehesive patient records, routine prescription refills and up-to-date and timely prescription status communications.
Work with payors to navigate benefits investigations, provide prior authorization and reauthorization support and resolve reimbursement challenges. Assist patients in understanding their insurance benefits and advocate for financial assistance and/or copay support when available.
Liaise with manufacturer hubs to obtain necessary documentation, prior authorization support, and financial assistance for patients. Ensure timely delivery of prescriptions and monitor and/or report on hub required metrics.
Compile and provide feedback to manufacturers on patient experiences, medication efficiency, adherence and other required metrics. Participate in manufacturer-sponsored training and education programs to stay current with industry developments. Maintain accurate records of REMS-related activities and ensure adherence to these protocols.
Positive coordination with the compounding, retail, logistics, and marketing departments is required.
Reporting to Specialty Pharmacist, PIC or their designee in a prompt, efficient manner with the proper disposition is expected. Continued training as well as participation in routine departmental or organizational meetings will be required.
Other projects, activities, or responsibilities assigned at the discretion of the partners or their designee.
Minimum Qualifications
Minimum two years experience in a health-care setting or customer-service is preferred.
Computer skills that include word processing, and efficent use of the internet and e-mail.
Excellent communications skills and organizational skills.
Ability to work independently and as part of a multidisciplinary team.
